Note: This root cause analysis (RCA) is from real-time scenarios that happened in industries during the tenure of two or three decades ago. These articles will help you to improve your troubleshooting skills and knowledge.

Burst Transformer Problem

An 11000/400V and an 11/3.3KV transformer burst – a pre-SC treatment & breakers reliability enhancement era problem.

Why burst breakers?

System insulation resistance (IR) loss common in the pre-SC treatment era caused phase-phase short several rated fault currents.

The transformer secondary breaker did not trip within 30 mS (milli-second) as expected though the protections relay commanded in time. The too-long current flow burst the breaker and the transformer.

Such problems vanished after the Silicone Compound (SC) application and Switchgear mechanical improvements described above.
